1. What exceptions can be made to waive
the program’s non-union requirement for eligibility?
No
exceptions can be made.
2. Is Southern New York Association
(SNYA) a nursing school or nursing program?
SNYA is NOT a nursing
school/program. SNYA sponsors students to attend nursing programs.
3. Is there an age requirement for the
LPN program?
No. However, the applicant has to have either a high
school diploma or a GED and also has to have at least 6 months
experience
in the health care field. (Proof: pay stub, W-2 form, letter from
health care employer).
4. Can my address on my documents be a
P.O. Box?
No. All documentation must have a street address
and all documents must match a valid New York State ID.
5. What if I don’t have any utility
bills in my name? (They’re in spouse’s, parent’s, sibling’s name).
We can
accept utility bills as long as they have the same address as your
photo identification.
6. Is there an application fee?
No.
There is no fee to apply to this program.
7. How long is the program?
Each nursing
program has different time frames and different start dates.
8. If I am already in an LPN school or have
been accepted to begin in an LPN school, am I eligible?
Yes. Please apply
via the website and follow instructions. Also,
please fax in your school acceptance letter and
orientation letter.
9. How do I submit my completed
application and documents?
You can either mail or fax ALL REQUIRED INFORMATION
to our office. We recommend you fax the
information since it is a time sensitive program. The SNYA fax number is (212) 968-7710.
10. How do I know if my paperwork was received by
your office?
We recommend calling our office, (212) 425-5050 a
half hour if you have faxed it or a few days after you
have mailed it. We will then be able to inform you
if all required documents were received.
11. What will happen if I don’t submit
all of the information requested?
The application will not be processed
and the paperwork will be shredded.
12. Once I have submitted the required information,
what do I do?
This is a grant funded program and all activities are
time sensitive. Once SNYA receives all required
documents, they will be reviewed for program
eligibility. Only then will candidates be notified
to come in for assessment and discussion. You will
also need to bring in your original diploma for
verification.
13. Can you send me an application? (My
printer doesn’t work or I don’t have a printer).
The program has been
designed to have applicants take responsibility for downloading the
application from the internet, making individual arrangements for
printing documents, and completing and submitting the required
paperwork via fax. Future nurses are encouraged to check with a
local library for access to the internet and printing.
14. Do you have any other nursing school programs
besides LPN? (i.e. RNs, CNAs).
At this time, we only offer the program for LPNS.
15. What if I misplaced my high school diploma or GED?
It is the applicant’s responsibility to secure a
duplicate document from the original source. We can not
accept letters from the school verifying your graduation
from that school.
16. What do I do if my current license, NY State ID,
or permit does not have my current address?
You will need to go to the Department of Motor
Vehicles and complete the necessary paperwork to change
your listed address. In the interim, you can send
the temporary information.
17. I received my high school education in another
country. What documents can I submit?
Please submit a diploma, transcript and proof of
authentication of documents from the organization
evaluating credentials recognized by the school of your
choice.
18. What other documents should I submit?
(Certificates of Achievement, Resume, Letters of Recommendation)
Please submit only the documents requested on the check
off list. Any unnecessary paperwork will be
discarded.
19. If my phone number does not appear on the bill,
what should I do?
Go to your carrier's website and download your
bill. Please only send the first page with your
name and address and any other page that has your phone
number on it.
20. Do I get to choose the school I want to go to
once I am in the program?
Yes, but first you must pass their entrance exam.
21. I do not see my question above. How
